Customize the Intake Form for a Service
You can set a default intake for scheduling in your main account settings. vcita also allows you to add additional questions to the intake form on a per-service basis. For example, if clients are booking massages, you may want to find out ahead of time what music and scents they prefer. However, you probably don't need to ask them about these preferences when they reserve spots in a stress-reduction workshop.
Add a Service-Specific Intake Field
- In your vcita account, navigate to Settings > My Services.
- Click on the service you want to ask additional questions for.
- Scroll to the bottom of the settings page where you will see the Client intake form section. Click Add/Edit.
- Click Add Field and select the field type from the dropdown.
- Give the field a label (or the question which will be asked) and configure it based on what you selected.
- Click on the Required checkbox if needed. Clients won't be able to submit the form without filling in the field if enabled.
- Click Save to add the field.
- Click Save at the top of the page to save your changes to the service.
Edit/Delete a Service-Specific Intake Field
- Navigate to Settings > My Services.
- Click on the service you want to ask additional questions for.
- Scroll to the bottom of the settings page where you will see the Client intake form section. Click Add/Edit.
- Hover over the field you need to change and click the pencil icon to Edit, or click the trash can icon to delete it.
Client Experience
The service-specific intake questions appear at the end of the form, after your default scheduling form questions.
Staff Experience
When you receive a booking, the extra information will be submitted along with the booking. You will see it on the appointment details page, as well as in the conversation in the Inbox.
Please Note: While the additional info will always be contained within the individual booking, any information collected using service-specific intake fields is not saved to the main client card. If the information must be saved long term to the client card, consider adding the field to your default intake form or manually add the information as an internal note on the client card.
